MDS Nurse (LVN) – Part‑Time

About the role

The MDS Nurse (LVN/LPN) is responsible for completing Minimum Data Set assessments in a long‑term care or skilled nursing facility. This part‑time position ensures accurate resident documentation, supports Medicare/Medicaid reimbursement, and works closely with interdisciplinary teams to promote quality care.

Key responsibilities

• Complete MDS assessments and Care Area Assessments within required timelines.
• Maintain accurate clinical documentation to support care planning and reimbursement.
• Ensure compliance with CMS, state, and facility guidelines.
• Collaborate with nursing, therapy, social services, and dietary teams to gather resident information.
• Participate in resident care conferences, quality improvement initiatives, and audits.
• Provide education and guidance to staff on documentation standards and MDS requirements.
• Communicate with residents and families regarding care plans.

Required profile

• Current LVN/LPN license in the state of practice.
• Prior experience in long‑term care or skilled nursing facilities preferred.
• Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines.
• Strong organizational skills with attention to detail.
• Ability to work collaboratively with interdisciplinary teams.

Required skills

• Proficiency with Microsoft Office Suite.
• Experience using electronic health record (EHR) systems.
• Familiarity with MDS 3.0 documentation tools.
